Age related macular degeneration is a multivariable disease, and
with the newest diagnostic tools, new variations of the classic
neovascularization have been recognized such as “Idiopathic
Polypoidal Choroidal Vasculopathy” (IPCV) and “Retinal
Angiomatous Proliferation” (RAP) that correspond differently in the
various therapeutic approaches.
Combo treatments (combination of anti-VEGFs, steroids and/or
PDT) gain ground lately. At

Combined (Combo) Treatments
Patients with wet ARMD and a neovascular membrane in the fovea
rarely experience an improvement in vision, even after confronting
the neovascularization.
Some of these patients are good candidates for surgical intervention.
These are patients with wet ARMD that have lost central vision in
both eyes, but one eye suffers for less than six months.

Surgical Treatment
compounds, that block and destroy pathological new vessels thus
stopping the fluid and blood leakage that is harmful for vision.
PDT has an advantage over Laser Photocoagulation because the
laser used is milder and does not cause any thermal damage to
the macula. It is also a simple procedure that does not need any
hospitalization. Due to the interaction that the drug has with sunlight,
the patient is advised to avoid sunlight for 48 hours
The retina with its photoreceptors is detached from the diseased
with neovascularization Retinal Pigment Epithelium and then is
placed in an adjacent area where the RPE is healthy. The transposition
of the macula can be superiorly, inferiorly or nasal with different
techniques depending on the extent of the section of the retina.
Total translocation of the macula offers the largest migration range.
